New Patient Registration


Please note: some people who have come to the U.K. from abroad may be eligible for NHS GP services but may not be eligible for NHS Hospital or Secondary care. It is your responsibility to confirm your status.


If you need to register pLease attend the surgery between 14:00am – 15:00 pm Mon, Wed, Thurs & Fri.  The staff will be pleased to process your registration. 

As part of your new registration, you will require to have a health check appointment with the Health care Assistant or a nurse

It is highly recommended that you have this first health check appointment for assisting the doctor for your future treatment.

If you are in the area for less than 3 months you will need to complete a special form for temporary residents.


If you leave the country for more than 3 months, you will be removed from our list.


Please note: you will be registered with Dr M C Patel who is a male doctor.  We have made provision for a female doctor  if you wish to see one. We have a female doctor available most days.
