We are happy to register new NHS patients within our practice boundary area. Please click the link below to see if your postcode is in our boundary area.
How to Register
To register with our practice please follow the link below to download the registration form. One must be completed for each person wishing to register.
As well as the registration form, you will need to fill in one of the Patient Data Forms below. One must be completed for each person wishing to register. Please complete and email to BRECCG.firstname.lastname@example.org before coming to the practice with your ID.
Once you have printed and completed the forms, please visit the surgery within 7 days with two forms of identification.
Acceptable identification includes:
- Birth Certificate
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration / mail forwarding letter.
It has always been the practice policy to ask for proof of address so we can check that you live in our practice boundary area.
To establish who you are and in order to enter the registration details correctly i.e Name, Date of Birth and Town and country of birth, you will be required to produce PERSONAL identification.
WE WILL NOT REFUSE registration or appointments because you do not have a proof of address or personal identification at hand.
As part of your new registration, you will require to have a health check appointment with the Health care Assistant or a Nurse.
It is highly recommended that you have this first health check appointment for assisting the doctor for your future treatment
We will contact you to book the registration health check once we have received all you documents.
For children’s registration it would be helpful if their immunisation details are e-mailed with registration forms.

Registering for Online Services
You can use the Internet to book and cancel an appointment with a GP, request repeat prescriptions and view aspects of your medical record online.
